Why Teen & Young Adult Acne Happens

Teen acne is, at its core, a hormonal story. During puberty, the body produces a significant surge in androgens, hormones that include testosterone, in both males and females. Androgens directly stimulate the sebaceous (oil) glands to produce more sebum. More sebum means more material to clog pores. And clogged pores, combined with naturally occurring skin bacteria, create the conditions for breakouts.

Here's the sequence inside the skin:

  1. Androgens surge during puberty, signaling sebaceous glands to overproduce oil.
  2. Excess sebum mixes with dead skin cells (which don't shed normally in acne-prone skin) inside the hair follicle.
  3. The follicle becomes blocked — creating a comedone (blackhead or whitehead).
  4. The bacteria Cutibacterium acnes, a natural resident of the skin, thrives in the oxygen-deprived, oil-rich environment of the clogged pore.
  5. The immune system detects the bacterial overgrowth and launches an inflammatory response — producing the red, raised, painful papules, pustules, and cysts of inflammatory acne.

Teen acne tends to show up most prominently on the T-zone, forehead, nose, and chin as well as the cheeks, jawline, back, chest, and shoulders, where sebaceous gland density is highest. In young adults in their 20s, the hormonal driver may ease somewhat but lifestyle factors like stress, diet, sleep, comedogenic products take over as primary triggers.


The Six Triggers That Make Teen & Young Adult Acne Worse

01   Hormonal Surges

The androgen surge of puberty is the primary driver of teen acne. Breakouts often worsen around hormonal events, for young women, this includes the menstrual cycle, with flares common in the week before a period when progesterone peaks. Teen Acne: Your Questions Answered

At what age should teens start using active skincare for acne?
